What is SOAP note?
A SOAP note is a clinical documentation format with four sections: Subjective (what the client reports), Objective (what the provider observes and measures), Assessment (the provider's evaluation), and Plan (the treatment performed and what comes next). Aesthetic clinics use it, or structured variants of it, for treatment records.
How it works in an aesthetic clinic
For a filler appointment: S records the client's goal and any history changes; O the examination, photos, and product lot number; A the candidacy call and approach; P the product, volume by area, technique, aftercare, and follow-up. Structured fields for units, sites, and lots make notes queryable later: every client who received a recalled lot, found in one search.

Why it matters for clinic operators
The chart is the clinic's memory and its legal defense, and format is what makes it usable as either. A structure the whole team follows also survives provider turnover: the next injector reads the record, not the previous injector's mind.
